By submitting, you have read, understand, and agree to the below:

 

The Contact Information provided is correct and vaild.  This information will be used by the Coaching Staff for sending communications throughout the Season

 

MEDICAL AUTHORIZATION: I/We, the parents/guardians of the above-named participant, hereby give my/our authorization for any emergency medical treatment of the participant for any injury resulting from any activity of the TDCJFL and/or including to and from such activity. It is understood that efforts shall be made to contact the undersigned or other emergency contact person as indicated above prior to rendering treatment to the injured participant. This authorization shall expire Dec. 31, of the year this application is signed.

 

PARENT/GUARDIAN AUTHORIZATION: I/We, the parents/guardians of the above-named participant, hereby give my/our approval for participation in any and all of the activities of the Touchdown Club Junior Football League (TDCJFL) during the current season. I/We assume all risks and hazards incidental to the conduct of any of the activities, including transportation to and from such activities. I/We do further release, absolve, indemnify and hold harmless the TDCJFL and/or its organizers, sponsors, directors, coaches and any or all of them. In case of injury to my/our child, I/we hereby waive all claims against the organizers, sponsors, directors or coaches appointed by them. I/We likewise waive, to the extent not covered by liability insurance, any claim against any persons transporting my/our child to or from activities.


EQUIPMENT RESPONSIBILITY: As parent/guardian, I/we do hereby assume full and complete responsibility for the proper care and maintenance of all equipment loaned to player, and agrees to return it on the designated date given to the player. I understand all equipment is to be used for TDCJFL activities ONLY and that all equipment remains the legal property of the TDCJFL. I agree to reimburse TDCJFL for any and all equipment that is lost, damaged or stolen for the full replacement cost of said equipment, with payment due when equipment is requested by the TDCJFL, or immediate upon the withdrawal of said player from TDCJFL. Failure to return equipment will result in a charge of $500.


PLAYER/PARENT CODE OF CONDUCT: I/We, the parents/guardians of the above-named participant, understand the Player/Parent Code of Conduct and agree to conduct themselves in a manner consistent with the TDCJFL’s Player/Parent Code of Conduct.

 

PHOTOGRAPH: I/We hereby consent and irrevocably authorize the TDCJFL to use at any time for the purpose of publication, business advertising or other purpose, reproduction of my son/daughter’s photograph or videotape together with their name, and I/We hereby waive any claims.
