IMPORTANT NOTICE 

The following is required to be submitted BEFORE equipment will be issued or contact practice can begin. NO EXCEPTIONS!!

• Copy of Birth Certificate

• Copy of Current Insurance Card / Medical Card. If Covered By Medical Card, A New Card Must Be Given Each Month Of Play (Current Year)

• Copy Of Current Physical (Coverage For July To November)  

• Completed Online Registration 

• Registration Fee

• Signed Copy of Player Code of Conduct Form

• Completed Medical Authorization Form
